Clearance Forms - must be submitted yearly
Submit your athletic clearance forms online! 

A reminder that you must print out the last page of the online form (complete with doctor’s clearance) and submit to the Athletic Director.

If you do not have internet or printer access you may pick up hard copies at the Athletic Directors Office, Wood Hall #103 –or- from the Principal’s Office.
 
All incoming freshman participating in a Fall sport must submit a copy of their final 8th grade grades to verify academic eligibility.  Academic eligibility is a 2.0 gpa or higher.      
 
1.ONLINE OPTION:  Follow this link and be sure to print out the last page and return it with doctor's signature to the Athletic Director.
