Volunteers
ALL POTENTIAL VOLUNTEERS/CHAPERONES MUST FILL OUT A PACKET
Thank you for your interest in becoming a chaperone or volunteer with the Tamalpais Union High School District! You must complete the requirements below and submit your volunteer application to your contact at the school site. Any questions regarding the volunteer packet should be directed to Suz Blunk in the Human Resources Department at sblunk@tamdistrict.org.
PLEASE INCLUDE THE FOLLOWING IN YOUR EMAIL:
- Which Site/School You're Supporting
- Which Dept/Teacher/Athletics Team
- Are You Volunteering On Campus or as a Chaperone For Field Trip Support
- * Note: Chaperones who are attending overnight field trips contact sblunk@tamdistrict.org regarding Keenan Training Requirements. *
- The Name Of The Staffer/Teacher You Are Working With
VOLUNTEER PAPERWORK
Once you connect with Suz Blunk, they will provide the following instructions/paperwork for you to complete the following
- Volunteer Application
- Fingerprint Clearance: Upon receiving a copy of a form of ID from you, we will provide a fingerprinting form and instructions on where to be
fingerprinted at no cost to you. - TB Clearance: Contact the District Nurse, Nancy Griffin (formerly Parnow) informing her your need for a TB assessment.

- The school/location you support
- The name of the teacher/coach/staff member you are volunteering with

We accept TB results up to 4 years. If you have one that is still current, please provide a copy of the test that shows your name and the negative test result. If the clearance is close to expiring, you will need to complete a new one. You can also use your own medical provider if that works better for you, but the results will need to be certified by a medical doctor and include your name and date of birth on the results.
* Note: Chaperones who are attending overnight field trips will need to complete state mandated certifications. *
