PLEASE REPORT ALL WORK-RELATED INJURIES!

Step 1: If you sustain a work-related injury, contact your Supervisor, and call the Cariva Care Line together:

A Care Operator can be reached at this number is available 24/7 to assist you with first aid and/or directions for seeing a doctor at the Kaiser On-the-Job occupational health facility in Terra Linda, if needed, and for reporting a workers’ compensation claim, if necessary. 

Step 2: Notify HR after you've reported your injury. You can leave a voicemail at 415 945 1030 or email DOHumanResources@tamdistrict.org

Step 3: Complete the Workers Compensation Claim Form and return it to HR to be signed immediately.
